PREVENTIVE CARE CHECKLIST APP
Objective
Proof of concept - To create a health plan member facing application that can be accessed from desktop or mobile devices where members can stay updated on preventive screenings/immunizations recommended for the age/gender/current health condition to manage their health better. The goal is to close the gaps in care and getting members to actively engage in their health management.
Solution
Created to-do item as a card where each are organized by general category by age/gender, other rows for disease specific groups with completion percentage to encourage members to take actions to raise the completion level. Each card shows due date and quick summary of provide explanations on why this is important for the user.
