Risk Adjustment and Hierarchical Condition Category coding is a payment reimbursement model for Medicare and Medicaid. Primarily designed for hospital administrators and billing staff to identify potential reimbursement opportunities by checking assigned codes against physician's notes and documentations, and suggests appropriate code with RAF (Risk Adjustment Factor) gain.
